Health Questionnaire
By filling out the questionnaire, you allow us to assess whether the requested therapy is suitable for you and to prepare thoroughly for your initial consultation. The more detailed your answers, the better advice we can provide. Required fields are marked with an asterisk.
If you do not book any appointment with us after the questionnaire evaluation, all your data and the entire health questionnaire will be removed from our system.
Personal Data Protection (GDPR)
Your data is processed exclusively for the purpose of assessing therapy suitability and preparing the initial consultation, in compliance with GDPR regulations.
If you do not book any service within 5 days of the questionnaire assessment, your questionnaire and all related data will be permanently deleted from our system.
If you do book an appointment, after the initial consultation we will either create a client card for you (with your consent), or delete your data as well.
